You need to complete this form for every course you take part in face-to-face, on a webinar or through e-learning as it is how we show the Welsh Government and UNISON that we’re delivering on our publicly funded learning project and reaching our members across Wales.
It also allows the Welsh Government to contact you for evaluation purposes. Please read the UNISON and Welsh Government Privacy Notices.
